Many Canadians have hypertension, diabetes, heart failure or high cholesterol; these are defined as chronic diseases. Optimizing the health of chronic diseases is a partnership between you and your family doctor. Together we will continue to work with you on your health and provide you care, despite this pandemic.

Hypertension (High blood pressure)

To help us help you, we ask that our patients diagnosed with hypertension obtain a blood pressure machine to monitor their blood pressure from the safety of their home. Studies show that home monitoring of your blood pressure leads to more ideal management of hypertension. If your blood pressure readings are above the target blood pressure suggested by your doctor, please contact our office.

As select private insurance companies will provide coverage for a blood pressure machine, we recommend looking into your insurance plan to determine if this applies to you.

Diabetes

To help us help you, we ask that our patients diagnosed with diabetes obtain a blood pressure machine to monitor their blood pressure from the safety of their home. In addition, please continue to monitor your blood glucose levels with your glucometer. We ask that you continue to go for blood work when it is recommended by your doctor.

As select private insurance companies will provide coverage for a blood pressure machine, we recommend looking into your insurance plan to determine if this applies to you.

Don't forget to regularly monitor and check your feet at home for any signs of wounds or skin areas that aren’t healing!

Heart Failure

To help us help you, we ask that our patients diagnosed with heart failure monitor their weight on a daily basis. In addition, please obtain a blood pressure machine to monitor your blood pressure from the safety of your home. Please keep in mind local laboratories are open. We ask that you continue to go for blood work when it is recommended by your doctor.

Dyslipidemia (High Cholesterol)

To help us help you, we ask that our patients diagnosed with dyslipidemia obtain a blood pressure machine to monitor their blood pressure from the safety of their home. We ask that you continue to go for blood work if that has been recommended by your doctor.
